- Have you previously completed Nurse Aide I training?
- In which state did you complete your Nurse Aide I training?
- Has your CNA license expired within the last 2 years?
- Have you taken a CNA class within the past 2 years?
- Are you or were you a military Nurse? a. If yes, please provide supporting documentation.

Important Payment Reminder
If you choose an installment plan, please note that your first payment must be made before classes begin, and your final payment is due no later than the end of the first week of class. Failure to make payments or to follow through with the agreed payment arrangement, as outlined in our policy, may result in removal from the course.
Note: The total tuition cost for the Weekend Accelerator Course includes the State Board Exam fee.
